ABSTRACT
32 year-old female, with intermammary lesions secondary to sinus pilonidal, scars and fetid exudates. The
physical exam showed acne conglobata with an active inflammatory component in trunk and gluteal
region, suppurative hidradenitis and extensive follicular affection due to dissecting cellulitis. This is an
exceptional presentation of a sporadic syndrome or follicular occlusion tetrad condition, with a complex and
multidisciplinary treatment. This is a clinical variant, because of age, gender and insidious presentation, whose
prognosis, despite the varied therapeutical measures, is still uncertain.
