Combination therapy to close wounds secondary to compartment syndrome of the foot. Report of two cases

ABSTRACT
Wounds secondary to compartment syndrome may be due to the dermofasciotomy which is carried out for decompression, or cutaneous necrosis for an untreated syndrome. We present two cases of patients that, after foot trauma, developed a compartment syndrome. Delayed diagnosis and treatment that caused chronic wounds produced an extended area of necrosis and tendon exposure; both had comorbidities, which increased the severity of the injuries and hampered reconstruction. In both of these cases, treatment was initiated with excision of the devitalized tissue, preserving the tendons. Wound treatment was carried out with a combination therapy: using negative pressure therapy and local treatment with alginates, antimicrobial and silver patches. After surgical cleaning, these patients were handled as outpatients; none of them had problems with the care of their suction devices. Tendons were preserved and the size of the wound was decreased. In one, total wound closure was achieved and in the other one closure was achieved with the application of cutaneous skin graft on the newly formed tissue. The skin cover obtained was good quality and did not require another reconstructive procedure. Combined therapy of the local treatment and the use of negative pressure therapy is a very useful alternative for the treatment of chronic wounds difficult to close secondary to compartment syndrome, helping maintain the exposed anatomical elements and contributing a quality cutaneous cover.
