Rizartrosis treatment with Eaton’s arthroplasty

ABSTRACT
Trapeziometacarpal osteoarthritis affects 10% of middle-aged women and it is more common in postmenopausal women. It is a degenerative disorder of the articular cartilage, which is the second joint most frequently affected. It is caused by the weakness of the deep palmar ligament, which normally prevents anomalous dorsal displacement of the first metacarpal in various movements. Pain is the first symptom; it appears with the thumb fully abducted and also when doing the lateral clamp. As the disease progresses it may present limitation of the thumb’s abduction and a radial deformity on the edge of the joint, due to the subluxation of the first metacarpal bone. Eaton distinguished four degrees of the disease, and each with a corresponding surgical treatment. The experience with 58 hands of different degrees is presented. On patients with grade 1 ligament reconstruction was performed, and on those with grades 2, 3 and 4 partial trapeziectomy, ligament reconstruction and tendinous interposition was carried out respectively. After three months and a year the pain and symptoms disappeared in 54 cases (93.86%). Only one case had to be reoperated.
