Pharmacological techniques for haematic saving

ABSTRACT
Blood conservation is critical in the perioperative practice of the anesthesiologist; its goal is to reduce or eliminate allogeneic blood transfusion and blood products. Blood is a scarce resource, and blood transfusion has potential immunological and non-immunological adverse effects. Besides blood transfusion is associated with increased mortality in cardiac surgery and increased linearly related to the number of units transfused. These represent significant economic costs associated with allogeneic transfusion significantly impacting health systems. Therefore knowledge of blood conservation techniques including hematological sparing drugs available in the perioperative is essential for proper anesthetic practice. Drugs that have been widely used and studied include aprotinin, the antifibrinolytic (inhibitors of serine protease derived from bovine aprotinin, tranexamic acid and aminocaproic acid, both lysine analogues), desmopressin (synthetic analogue of antidiuretic hormone), factor VII recombinant activated and prothrombin complex concentrate. This review aims to inform the medical community in general, but especially medical specialists in anesthesiology, current concepts on the use of pharmacological hematic savers.
