The sign of “spread ink” in the melanotic nevus. Report of 10 cases

ABSTRACT
Background: Nevus are localized skin malformations. Melanotic nevus can be acquired (common melanotic nevi) or inherited; they can also appear in syndromes such as neurofibromatosis with the café au lait macules.Objectives: To report a clinical feature of the melanotic nevus that could help with its diagnosis when seen in other syndromes.
Methods: We studied 10 patients with melanotic nevus, and 5 with neurofibromatosis type 1, with 45 café au lait macules.
Results: Every patient with melanotic nevus showed a prolongation of the macule similar to spread ink (the sign of spread ink). None of the 45 café au lait macules of the 5 patients with neurofibromatosis type I presented this sign.
Conclusions: We consider that the appearance of this clinical feature, “sign of spread ink”, makes the diagnosis of melanotic nevi easier, by ruling out the café au lait macule of the neurofibromatosis.
