Dermatitis herpetiformis with immunofluorescent fibrillar pattern: Case report and literature review

ABSTRACT
Dermatitis herpetiformis (DH) is a rare blistering, autoimmune
disease of multifactorial etiology, deemed to be a cutaneous
manifestation of dietary gluten hypersensitivity. Direct immunofluorescence
(DIF) of perilesional skin usually reveals a granular
pattern in the upper papillary dermis or the basal membrane.
However, there are reports of a rare fibrillar pattern that has
been associated with atypical DH presentations, negative serology
and even absence of the of HLA-B8/DR3DQ2 haplotype. This
is a report of a typical DH case with fibrillar DIF findings, including
a brief review of existing literature.
