Renal attack. A proposal of a new concept

ABSTRACT
Acute kidney injury is common with an incidence of 2.1/1,000 persons,
similar to that of acute myocardial infarction, and is a major independent
risk factor for death in the critically ill patients. Acute kidney injury is
today a facing a complex phase of characterization due to a lack of an
accepted definition. In practice the diagnosis of acute kidney injury
has so far been made based on changes in serum creatinine or urinary
output, confounding between concepts of kidney injury and kidney
dysfunction. On the other hand, different names have been used to
define this disorder, such as acute tubular necrosis, acute renal failure
or acute kidney disease, which creates more confusion and delayed
diagnosis and treatment. For this reason recently several consensus
have been developed to define this entity, which resulted in RIFLE and
AKIN criteria and KDIGO guidelines, including different biomarkers.
Similar to concept of heart attack has been proposed renal attack for a
comprehensive and early assessment of acute kidney injury. The aim
of this paper is to propose the concept of renal attack in the critically
ill as a new tool for assessing acute kidney injury.
