Gut flora and energy metabolism after bariatric surgery

ABSTRACT
In a world besieged by the epidemic of obesity,
bariatric surgery has emerged as an alternative
for controlling the progression of this comorbidity
towards the Metabolic syndrome
complications such as Diabetes mellitus and
arteriosclerosis. Gastric bypass followed by a
Roux-in-Y jejuno-jejunostomy, has been the most
effective bariatric procedure in the control of
complications associated with obesity. Bariatric
surgery produces changes in the anatomy,
motility and functions of the digestive tract;
modifies the paracrine enterohormonal pattern,
and reduces the obese´s food intakes. All these
changes lead to the reduction of insulin
resistance, and hence, a better use of glucose by
cells, tissues and organs in the periphery, return
of serum glucose to normality, and diminishment
of glycated Hemoglobin values. Bariatric
surgery also affects bacterial composition of gut
flora, and it is believed that this event might also
help in a better use of glucose by the periphery.
Results of experimental models in animals,
combined with observations of bariatrized
patients, have led many to suggest that gut flora
may exert an important role in the development
of Metabolic syndrome, and also, contribute
with its treatment once bariatric surgery is
performed.
