Lymphoscintigraphy with sentinel node biopsy in penile carcinoma, preliminary results

ABSTRACT
Purpose: We showed the technique and assessed the usefulness of dynamic sentinel node biopsy in staging patients with carcinoma of the penis in the Hospital de Oncología del Centro Médico Nacional Siglo XXI.
Materials and methods: From 2003 to 2005, 31 patients were treated, 6 of which were managed with dynamic sentinel node biopsy. We assessed the procedure retrospectively.
Results: In all patients were identified the sentinel node by lymphoscintigraphy preoperatively and intraoperatively by the gammaprobe and by the blue dye. The transoperatively histology report of the sentinel nodes was concordant in all the cases with the definitive histology. The mean age was 67 years with a median of 63 (52-90). Squamous cell carcinoma was the most common histopathological report in 83% (5 patients), the remainder was carcinoma papillary (1 patient) 17%. Two patients were treated with total penectomy (33%), three with partial penectomy (50%) and the remainder with wide circumcision (16.7%), as primary treatment.
Conclusions: In our experience dynamic sentinel node biopsy is a useful clinic tool for prognosis and staging for patients with penile carcinoma without palpable inguinal nodes on the initial physical examination. It helps avoiding to perform a conventional inguinal lymphadenectomy and it’s complications. Perhaps penile carcinoma is an uncommon malignant disease, with a low incidence, is necessary a large number of patients to validate this study.
