Extended-spectrum beta-lactamaseproducing Klebsiella pneumoniae and Escherichia coli isolated from patients with health care-associated infection in a university hospital

ABSTRACT
background. Health care-associated infection (HCAI) caused by extended-spectrum beta-lactamase- (ESBL) producing
enterobacteria, represent an emerging public health problem. In this study we determined the presence
and distribution of
blaESBL genes in
K. pneumoniae and
E. coli strains isolated from patients with HCAI at The Andes
University Hospital (TAUH), Mérida, Venezuela.
materials and methods. Fourteen
K. pneumoniae and 12
E. coli strains isolated from patients with HCAI in
TAUH collected from March to July 2013 were studied. Susceptibility tests were performed by minimum inhibitory
concentration and the presence of ESBLs was detected by the double-disk synergy test. Transfer of ESBLs determinants
was performed by conjugation and characterization of the
blaESBL genes by PCR.
results. Pneumonia was the most frequent diagnosis of HCAI (30.8%). All strains were ESBL producers, most
associated with resistance to quinolones and aminoglycosides. All ESBL determinants were transferred by conjugation.
The
blaCTX-M-1 gene was the most prevalent in
K. pneumoniae (50%), and
blaCTX-M-8 in
E. coli (50%). The
association of different
blaESBL genes was observed in 46.1% of strains.
conclusion. To our knowledge, this is the first description of
blaCTX-M-8 gene in
E. coli associated with
blaTEM and
blaCTX-M-9
genes in Venezuela.
