Acute renal insufficiency in patients under prolonged invasive artificial ventilation

ABSTRACT
Introduction: The acute renal insufficiency (ARI) is a frequent complication in the severe patient care ward, especially in those with artificial ventilation.
Objectives: To characterize the syndrome of acute renal insufficiency in patients with lengthy ventilation.
Methods: A analytical and cross-sectional study was conducted in all patients aged over 18 in critical status due to ARI and lengthy invasive ventilation admitted in the ICU for adult of the “Dr. Luís Díaz Soto” Hospital from January, 2004 to December, 2008. The ARI was correlated with demographic, qualitative and quantitative variables and each three days as system the SOFA index was determined for the diagnosis of the syndrome of multiple organ dysfunction.
Results: The patient who during their course had an ARI syndrome had a higher mortality, which was more evident in the clinical and traumatic category influencing in a lengthening of stage and ventilation time. Is significant the relation of predisposing factors mainly the sepsis and the hypovolemia with the increase of mortality (42 %).
Conclusions: The acute renal insufficiency (ARI) is frequent in patients under ventilation, with a significant relation to mortality of them and that the kidney as dysfunctional organ increase very much the mortality in patients with multiple organ dysfunction compared with the dysfunction of other organs.
