Sclerosant therapy of lymphatic malformations in childhood: a systematic review

ABSTRACT
Lymphatic abnormalities are usually diagnosed in utero or in the first two years of life. Surgical excision is the treatment of choice with a recurrence rate of up to 50 % on incomplete extractions. Therapeutic alternatives include intralesional sclerosant therapies, although, its effectiveness is unknown on pediatric population. We present a systematic review of the literature conducted in main virtual libraries (PubMed, LILACS, IMBIOMED) evaluating clinical case reports of pediatric intralesional sclerotherapy. We classified resolution of malformation as: Successful (≥ 90%), partial success (≥ 50-90%) or low success or therapeutic response (‹ 50%). Were identified 13 publications and that included a total of 155 cases. Overall effectiveness of sclerotherapy on lymphatic anomalies reduction according to type of sclerosant was: OK-432 [60.8%], Bleomycin [47.4%] and «mixed therapy» (dioxiciclina, ethanol and sodium tetradecyl sulfate [20.6%]). Of the totally cases here reported the 37% derived on surgery. Based on reports and case series reviewed, we conclude that the most effective sclerotherapy is OK-432.
