Endoscopic management of iatrogenic bile duct injury. Seven-year experience

ABSTRACT
Introduction: Iatrogenic bile duct injuries are a rare contingency with very important clinical, economical, judicial, labor and social implications. Inappropriate decisions and treatments can trigger disastrous consequences for the patient’s health in the short or long term. The advent of laparoscopic cholecystectomy has been associated with an increased incidence of injuries as well as its prognosis. The objective of the study is to describe the experience in the department of General Surgery regarding diagnosis, treatment and management of injuries of the bile duct.
Material and methods: We performed a retrospective study in which we gathered information about patients undergoing ERCP for diagnosis and/or treatment of bile duct injury after a cholecystectomy at Western Medical Center from 2005 to 2012. We analized medical records of patients which underwent ERCP, performing frequencies and percentages for qualitative variables as well as the average and the standard deviation for quantitative variables.
Results: We found that 86% patients who underwent ERCP had healed and only 14% required definitive surgery with biliodigestive bypass.
Conclusions: The low mortality is essentially due to the early recognition of the lesion. The experience of the surgeon is still the most important factor to help prevent bile duct injury.
