Clinical examination in the patient with stroke

ABSTRACT
Objective: To focus on the main practical aspects for systematic execution of the initial history taking and physical examination in the patient with stroke.
Development: We conceive the initial history taking and physical examination of stroke in a standardized and problem focused view with the purpose of making quickly and precise assessments. Integrating classical and current approach the following main areas of anamnesis in stroke are reviewed: source of history, time of symptom onset, currents symptoms characterization, personal history and family history. After that we considered the identification of cardiovascular physical signs and other non–neurological signs for the full diagnostic assessment of patients with acute stroke. The emphasis was made in some aspects for selection of candidates for urgent therapies and identification of potential rule out conditions. Under the perspective of National Institute of Health Stroke Scale we reviewed the classical techniques for neurological examination of these main areas: level of consciousness, ocular movements, visual fields, facial motor function, extremities motor function, movement coordination, somatic sensitive function, language and speech, extinction and inattention. Moreover, is compiled the conventional scale definitions and punctuation method for improve Spanish communication between health professionals.
Conclusions: Is delineated the necessity of improve and standardization of techniques for clinical assessment of stroke.
