Risk factors: a challenge in the evaluation of febrile children

ABSTRACT
Introduction: the incidence of severe bacterial infection has increased in the past 60 years and is a cause of death in the pediatric intensive care units in the United States and some European countries.Objective: to establish the relationship between risk factors and the incidence of severe bacterial infection for each risk category proposed.
Methods: an analytical, case-control study was performed, which included 282 febrile children from 3-36 months of age, classified with the risk categories. Frequency behavior of severe bacterial infection in febrile children was evaluated and how this was related to the "risk categories", belonging to the 12 family doctor's offices of basic work group #1 of "Emilia de Córdova" Community Teaching Polyclinic of San Nicolás municipality, Mayabeque province, in the period from January 1st, 2010 until February 1st, 2011; tests of statistical significance were applied chisquare and bivariate correlation coefficient.
Results: children over 1 year of age were the ones who mostly concurred to consultation due to fever without focus, over half of them were assessed as Low Risk when distributing them according to the applied risk categories, the most frequent risk factors in low-risk children were: poor housing conditions, overcrowding, smoking habit, problems with water and associated chronic diseases.
Conclusions: it is concluded that severe bacterial infections predominated in male febrile children under one year of age, the group assessed as risk and high risk were the ones that most progressed to severe bacterial infection in high risk children almost all risk factors had high frequency.
