Is the conventional arterial pressure measurement adequate for diagnostics and classification of arterial hypertension?

ABSTRACT
Introduction: Diagnosis Hypertension is controversial since it depends on conventional blood pressure measurements. In the last 30 years other methods have been increasingly used to improve the reliability of hypertension diagnosis, such as: multiple measurements of blood pressure in a single day and self-measurement home blood pressure thus could reduce the white-coat effect, white-coat HBP or masked HBP.
Method: A review of literature on the topic was made.
Results: Current blood pressure measurement methods are influenced by multiple factors which can alter the accurate measurement; therefore is a great professional responsibility to arrive to a correct diagnosis of HBP. Although ambulatory monitoring of blood pressure is the gold standard to obtain circadian variability, the method to minimize diagnosis errors is still discussed because of the high cost of 24 hour ambulatory monitoring. In other hand, different studies report 12- 53,2% white-coat HBP prevalence and 10-50% masked HBP, those prevalence variations should take into account.
Conclusions: Conventional Blood pressure is the most widely and easiest to be used, however, it should be complemented with single-day multiple measurements, with self-measurement home blood pressure, if possible, with 24-hour ambulatory BP measurement.
