ABSTRACT
Achalasia is a motor disorder of the esophagus characterized by loss of esophageal peristalsis and lack of or incomplete relaxation of the lower esophageal sphincter. In 1674 was reported the first case of achalasia by Thomas Willis in a healthy patient who presented vomiting and regurgitation of food. Although the exact cause of this condition is unknown, there has been a loss of ganglion cells within the myenteric plexus the distal esophagus. The most common symptoms are dysphagia, regurgitation, weight loss and chest pain. The esophagogram shows dilated esophagus and image «beak» at the distal level, endoscopy esophagogastric show gastroesophageal junction close. Esophageal manometry is the main diagnostic method showing aperistalsis with increased lower esophageal sphincter pressure. The goal of treatment is to reduce lower esophageal sphincter pressure so that gravity can facilitate esophageal emptying.
