Amyand’s hernia through trocar site. Case report

ABSTRACT
Background: Amyand hernia is a protrusion of the cecal appendix
through the abdominal wall defect. Initially described in an inguinal
hernia, but has expanded the definition to include femoral and
incisional hernias.
Clinical case: Women of 68 years old, with a history of laparoscopic
cholecystectomy 12 years ago. With clinical symptoms of sudden
onset painful lump in right lower quadrant. The computed
tomography reported an incarcelada hernia abscess wall. Then,
in laparotomy was found an abscessed herniation from cecal
appendix due to defect in the abdominal wall measuring 2 cm
of diameter. Appendectomy was performed and primary closure
of the defect with non-absorbable material. The patient went on
health and was discharged on the fifth postoperative day.
Conclusions: If the Amyand herniation is an uncommon disease
evenmore the presentation of postincisional herniation with
laparoscopic trocars from 5 ml and an inflamed appendix within
the hernial sac.
The preoperative diagnosis is difficult; it must be based on clinical
suspicion, supported by imaging tests.
