Pulmonar segmentectomy in a child with necrotizing pneumonia

ABSTRACT
Necrotizing pneumonia is a rare disease in healthy children. It occurs as a complication of a non responding invasive pneumonia after 48 hours of treatment and is diagnosed by means of a chest computed tomography. S. pneumoniae is the most commonly involved ethiologic agent. Necrotizing pneumonia usually responds to prolonged antimicrobial therapy and controversies exist with regard of surgical treatment because of the risk of complications such as bronchopleural fistula. Nevertheless, when the pleural space is affected it may require drainage of parapneumonic effusion, pleural decortication or resection of necrtotic tissue. We present a case of a previously healthy one-year-old girl with necrotizing pneumonia complicated with empyema who underwent pulmonar segmentectomy and pleural decortication for her treatment. The purpose of this report is to aware that necrotizing pneumonia may affect healthy children and to have in mind the need of surgical treatment.
