Current management of neuropathic pain

ABSTRACT
Neuropathic pain represents the most common form of chronic pain. The presence of this pain, chronic by definition, leads to comorbidities that directly impact the quality of life of patients. Among some common complications, chronic pain is related to anxiety, depression, and sleep disorders.
Therefore, the treatment of chronic pain requires knowledge of its pathophysiological aspects in order to choose the best management. Two diseases stand for the most common causes of neuropathic pain: diabetic neuropathy and postherpetic neuralgia. This review focuses on treatments in these two conditions that have demonstrated greater efficacy in clinical trials, with recommendation of drugs of first and second line, with their respective doses and more frequent adverse events. Also, considering that a proportion of patients will not respond to monotherapy, the review provides recommendations about the combination of drugs to achieve a greater reduction in pain severity. Finally are some drugs that have not yet shown its benefit in the treatment of neuropathic pain, but probably in a near future they may represent an alternative in the limited group of drugs indicated for the treatment of neuropathic pain.
