Predictive factors associated to functional disability and death at 30 days in patients with acute ischemic stroke: Results from the RENAMEVASC Study

ABSTRACT
Background: Cerebrovascular diseases are the second
cause of death worldwide and 85% of deaths occurred
in developing countries as Mexico.
Objective: To determine
the predictive factors associated to short-term
poor outcome or death in patients with acute ischemic
stroke in Mexico.
Patients and methods: From the
RENAMEVASC registry (by the Spanish eponym: REgistro
NAcional Mexicano de Enfermedad VAScular Cerebral),
we analyzed 891 patients with acute ischemic
stroke requiring hospital care. Patients were classified
as cases (n367) when clinical outcome at 30 days
showed a Rankin score of 4-6 (severe disability or death)
and as Controls (n = 524) when they had a Rankin
score of 0-3 (good outcome). Demographics, clinical,
radiographic, and hospital complications variables
were compared between both groups. Results: Case
fatality rate was 19.6% and severe disability was
observed in 21.5% of cases. Variables that were
associated to poor outcome or death included:
hemispheric infarction (OR 14.4; p ‹ 0.001), medical
hospital complications [cardiac arrhythmia (OR 12.7;
p = 0.03) and pneumonia (4.6; p = 0.001)], and
admission to a unit care (OR 4.6; p = 0.002).
Conclusions: Forty percent of patients in this Mexican
population had poor outcome because severe
disability or death, being the main associated factors
the extension of cerebral infarction and the presence
of medical complications.
