ABSTRACT

Introduction: In the pathophysiology of migraine has been increasingly accepted hiperexcitability the CNS, which is why prophylactic treatment with anticonvulsants has indicated to improve quality of life. Objectives: To compare the efficacy and safety of Amitriptyline against Topiramate in migraine prophylaxis through clinical evaluation and know well the costs and benefits of both drugs. Material and methods: Prospective studies, comparative, cross, and randomized trial. We included ambulatory patients in the neurology service with established diagnosis of migraine according to the criteria and classification of the 2005 International Headache Society (IHS). Results: We included 20 patients, 2 of whom were men. It was initially allocated by lottery to 8 Amitriptyline and 12 to Topiramate. The frequency and intensity of attacks fell from the start, with no difference in effectiveness between the two, to start the second period saw a floor-ceiling effect. All patients presented with Topiramate paresthesias, significant effect on the decision to suspend treatment. Conclusions: Both Amitriptyline Topiramate as are drugs effective in the preventive treatment of migraine; Amitriptyline, was preferred by patients (cost-benefit), its security, longer-term effect and lower cost.There were not interest conflict because patients bought their own medicine.
