Oblique Le Fort I Osteotomy for the Treatment of Severe Maxillary Hypoplasia

ABSTRACT
Since the beginning of orthognatic surgery, there have been different modifications made to the surgical techniques, with the goal of developing an ideal surgical technique, which provides better functional and aesthetic results for the correction of facial deformities. In this modification, Le Fort I osteotomy design is made in an oblique fashion through all walls of the maxillary sinus in order to release the maxillary bone and fix it in a new more anterior and superior position. The same basis of the conventional Le Fort I osteotomy is used with the exception of making the osteotomies in an oblique manner, in an upward direction from posterior to anterior instead of a bone cut made parallel to the maxillary occlusion plane. Doing so gives a maxillary projection, which improves the aesthetics of the patient’s profile. To perform this technique, titanium miniplates and screws are used for rigid fixation of the maxilla in its new position.
