Ten years-work of a Trauma Unit: An analysis from Neurosurgery view

ABSTRACT
Introduction: Patients with neurotrauma require of special units to improve medical attention. Objective: To analyze
our neurotrauma results and their influences on our community.
Patients and method: Our unit attended a total of
2,838 neurotrauma patients. 601 patients (21%) had severe head trauma (SHT), 819 (29%) moderate (MHT) and 1,428
(50%) minor (MiHD). Medical management combined therapeutic protocol, early image studies and intensive surgical
treatment. Patients with severe head trauma were all neuromonitorized. Management allowed to apply this experience
to pediatric neurotrauma, complex neurosurgical disorders and cranial base surgery. Our Unit carried out an intense
educational activity and participated on an international clinical research.
Results: Unit admitted 99% of neurotrauma
patients. 98% of SHT were neuromonitorized. Mortality decreased in severe head trauma from 56 to 30%, on MHT
from 12 to 3% and on MiHT from 3 to 1.1%. Mortalities on pediatric neurotrauma and cranial base neurosurgery
decreased from 20 to 10% and from 12 to 8%, respectively.
Conclusion: Our Unit influenced on improving clinical
results of neurotrauma management and on the scientific and technological development of our community. Besides,
Trauma Unit was an important neurosciences school of our community in the last ten years.
