Topographical anatomy of oval foramen with application to trigeminal neuralgia percutaneous treatment

ABSTRACT
Introduction: Trigeminal neuralgia is a relatively frequent problem and its initial treatment is pharmacological, but in
the evolution, a high number of patients does not respond. For this reason they will require some type of invasive
treatments, like percutaneous techniques, which have a fundamental place because its very low morbidity and almost
null mortality.
Materials and methods: 21 dry cranial hemibases, in which the oval foramen dimensions and the
distances that separate it from carotid, minor round and anterior ripped foramina as well as the distance from the mean
line foramina were measured. Four reconstructions with 3D tomography were also made to patients without trigeminal
pathology, with the objective to identify oval foramen.
Results: Oval foramen measures in average 7.3 X 4.5 mm and it
is located 22.2 mm outside the mean line and 23.8 mm in front of external auditory conduct. Carotid, minor round and
anterior ripped foramina are located 13.4, 1-2 and 6.5 mm from the oval foramen, respectively. In all cranial base
reconstructions, the oval foramen was easily identified and it is possible to estimate its distance to the mean line and
other cranial base foramina.
Discussion and conclusions: Considering these measures and the radioscopic guide,
surgical time and complications in percutaneous techniques of trigeminal neuralgia treatment, can be diminished.
