Using the LightCycler® SeptiFast test in a newborn and an infant for the rapid etiological diagnosis of nosocomial sepsis by Candida parapsilosis

ABSTRACT
A new molecular biology technique was used simultaneously in two patients with systemic inflammatory response syndrome
and negative cultures who were given broad-spectrum antibiotics without a vis ble improvement. The first case is a patient
of 17 months who was admitted to the Pediatric Intensive Care Unit with a diagnosis of abdominal septic shock. In the last
episode had fever and persistent laboratory abnormalities and the cultures were negative on three occasions, SeptiFast was
requested as an alternative test with a sample of blood from the catheter in place and identified the genetic material of Candida
parapsilosis and
Klebsiella pneumoniae/oxytoca, and the catheter was removed, with adequate response from the patient
and discharged when the infection was solved.
The second case is a male patient of 16 days of life who was admitted at birth in infants service because of respiratory distress
and necrotizing enterocolitis, at 72 hours from the suspension of the antimicrobial treatment started with fever, abdominal
distension, leukopenia and thrombocytopenia. New cultures were requested and SeptiFast test with one milliliter of peripheral
blood, which identified Candida parapsilosis as the pathogen agent. The treatment was changed to amphotericin B deoxycholate
with improvement at 24 hours of the change.
