El by pass secuencial distal: una cirugía resolutiva en pie diabético con gangrena y lesiones TASC D

ABSTRACT
Objective: To report the outcomes of patients underwent sequential distal inframaleolar by pass indicated
by gangrene and extensive lesions of femoral popliteal and tibial vessels.
Patients and methods: Retrospective, longitudinal study from Excell database files, in a period of
5 years from June 2003 to June 2008, including patients with arterial occlusive disease, from femoral-
popliteal to tibial vessels Rutherford category 6, TASC D treated with sequential distal bypass
inframaleolar.
Results: From June 2003 to June 2008 we found 19 patients (17 %) of a total of 110 bypass operations.
Thirteen women and six men, 100% with diabetes; 100% with critical ischemia with a presentation
of 15 days to 4 months, two patients with only pelvic limb. The perioperative mortality was 0%,
limb salvage was 85% with a primary patency of 84% and two years and 5 years patency of 57%
(fourth deaths).
Conclusions: The highly specialized vascular surgery includes this type of technique and is still on
a par with endovascular surgery with differences in: a) be invasive, b) warrant a considerable degree
of surgical skill, c) and specially great value in our series, we consider a final surgery in patients with
diabetes and critical ischemia.
