Accesos vasculares en una Unidad de Hemodiálisis y la participación del cirujano vascular

ABSTRACT
Introduction: Few have been known of the participation ofthe vascular surgeon in a hemodialysis
unit, because he doesn't get involved completely in its operation; although the election and accomplishment
of the permanent vascular access, its pursuit, and the correction of complications, runs to
their position. The creation of multidisciplinary group allows improving results.
Objective: To analyze the participation of the vascular surgeon in the operation of a hemodialysis
unit, and the opportunities of improvement.
Material and methods: Observational, descriptive, retrospective and cross sectional study in patients
with treatment of hemodialysis by terminal renal disease, in the Hospital of Section 50 of the
S.N.T.E., in Monterrey, Nuevo Leon; from January 1st 2007 to December 31st 2007. Demographic
characteristics were evaluated and the conducted procedures.
Results: 26 patients were included, 38,5% female and 61,5% male, age average 55,9 years ± 13,49
(19 to 76). 64 procedures took place altogether, divided in 16 (25%) autologous a1teriovenous fistulas
(AVF), 10 (15.6%) prosthetic and 38 (59.4%) central venous catheters. Angiology and vascular surgery
department made the 100% ofthe autologous and prosthetic AVF and the 15.8% ofthe catheters.
The incidence of autologous AVF developed to the beginning of hemodialysis was of the 12.5%. The
prevalence of patients with autologous, prosthetic AVF and catheter to the cut was: 23.1%, 19.2% and
57.7% respectively.
Conclusion: The participation of the vascular surgeon in the hemodialysis unit is important. The
incidence and prevalence of autologous AVF, were smaller than international recommendations,
the prevalence of catheters was greater. This allows participating in multidisciplinary form to obtain these goals.
