Gum Elastic Bougie use for emergency tracheostomy ventilation. A purpose of a case

ABSTRACT
Introduction: The presence of tumors with evident commitment of the airway leads us to consider these patients as a difficult airway know or anticipated. These have great value for the extrinsic or intrinsic commitment of the breathing ways, deviation or partial obstruction, and commitment of vocal chords. Their emergency surgical approach is also hindered by anatomical distortion.
Clinical case: A 55 year-old patient is presented with tumors of larynx stage T4N2M0 that is announced for laringectomy and functional radical dissection of neck, during the replacement of tracheotomy tube, the entrance takes place by a false via, for finds the traqueostoma tumoral. A ventilatory emergency is presented that is resolved with the use of the Gum Elastic Bougie through the traqueostoma locating the air way. The patient requires again in the postoperative a tube replacement for obstruction and it is carried out again with the guide.
Conclusions: The Gum Elastic Bougie constitutes a valid alternative to the failure of replacement of tracheostomy tube, for loss of the anatomical way.
