Acygos arch cannulation and perforation by a double lumen catheter for hemodialysis

ABSTRACT
Objective: To report a case of azygos arch cannulation and perforation by a central venous catheter.
Design: Case report.
Setting: ICU of a private hospital, Mexico City.
Patient: A 46-year-old woman with severe metabolic acidosis due to acute aspirin overdose.
Interventions: Insertion of a double lumen catheter via the right internal jugular vein, hemodialysis, assisted mechanical ventilation, thoracotomy and reparation of azygos vein, blood transfusion.
Measurements and main results: After surgery and hemodialysis the course of illness was satisfactory and the patient was discharged to home.
Conclusion: Azygos arch perforation is a rare complication of central venous catheterization.
