ABSTRACT
Weaning failure represents a serious problem in the intensive care unit due to its high morbidity and mortality. Patients with chronic obstructive pulmonary disease and neurologic impairment are frequently predisposed. Weaning failure is multi-factorial. Hypothyroidism could play a role in ventilator dependence been responsible for respiratory center disturbances, diaphragmatic dysfunction, obstructive or central apnea, and pleural effusion. Hormonal replacement as the mainstay of treatment, could improve the chances of successful extubation.
