ABSTRACT
Traumatic injuries of the hand especially in the thumb causes various important alterations for the function of the hand and limb, reconstruction of the thumb is a challenge and there are various techniques for management. We report the case of a 40 year old male patient sustaining a crushing thumb traumatism with a metal press that caused a comminuted fracture of proximal and distal phalanges, neurovascular injury and later the presence of a necrosis thumb. As treatment we performed an amputation of the thumb distal to the metacarpophalangeal joint with reconstruction using a modified wrap-around flap of the thumb to hallux amended. Different scales were used to assess limb function, hand, thumb and foot. The treatment aims to obtain a functional thumb, with adequate sensitivity, aesthetic, acceptable range of motion press function, residual pain free, with adequate bone support and skin coverage.
