ABSTRACT

We present a case of a life-threatening anaphylactic reaction with bradycardia occurring in a 76-year-old man chronically treated with a topic beta-blocker, who underwent elective surgery for an inguinal hernia repair. The patient had a history of atopy, several surgeries and chronic urologic catheterization. Sudden bradycardia and cardiorrespiratory collapse occurred 20 minutes into surgery. Intravenous epinephrine was successfully administered, the procedure was cancelled and the patient was transferred to the intensive care unit. Recovery was uneventful. Results of serologic tests for allergy were elevated. Total IgE level was high, at 1,386 UI/ml (0-100 UI/ml), and RAST for latex specific IgE was elevated at 34.8 kU/l, suggesting an allergy to natural rubber latex as the cause of the anaphylactic shock.
