Anafilaxia intraoperatoria por látex: Reporte de un caso

RESUMEN
Presentamos el caso de una reacción anafiláctica severa con bradicardia durante una cirugía para plastía inguinal en un paciente de 76 años de edad tratado crónicamente con betabloqueadores tópicos. El paciente tenía una historia de atopia, de varias cirugías y cateterización urológica crónica. El problema se presentó 20 minutos después del inicio de la cirugía con presencia súbita de bradicardia y colapso circulatorio. El paciente fue reanimado con adrenalina intravenosa, el procedimiento se canceló y la recuperación fue sin problemas. En consulta externa de alergología se le practicaron exámenes seroinmunológicos encontrando elevación significativa de la IgE total hasta 1,386 UI/ml (valor normal 0 -100 UI/ml) y de la IgE específica para látex mediante RAST hasta 34.8 kU/l (normal de 0 a 0.34 kU/l), sugiriendo la alergia al látex como la causa del choque anafiláctico.
