Calibration and discrimination of APACHE II and APACHE IV

ABSTRACT
Objective: The aim was to determine discrimination and calibration of APACHE II and APACHE IV in critical patients of the General Hospital of Mexico.
Design: A comparative, descriptive longitudinal prospective cohort.
Setting: Intensive care units of different specialties in the General Hospital of Mexico.
Patients: A total of 546 patients were included and analyzed.
Interventions: None.
Measurements and principal results: Observed mortality was 40.8%, the predicted mortality by APACHE II and APACHE IV were 21.1% and 15.8%, respectively. The standardized mortality ratio for APACHE II and APACHE IV were 1.93 and 2.58, respectively. Calibration values of APACHE II and IV were 11.456 (p = 0.120) and 3.802 (p = 0.875), respectively. The discrimination (area under ROC) of APACHE II and IV was 0.770 ± 0.020 (IC
95% 0.730-0.810) (p ‹ 0.001) and 0.770 ± 0.021 (IC
95% 0.730-0.810) (p ‹ 0.001), respectively.
Conclusions: Both scores showed good performance in the intensive care units of the General Hospital of Mexico for predicting mortality.
