ABSTRACT
With the increase of the elderly in the general population, more and more aged patients will be referred for surgical therapy. Age per se should not be used as a criterion for deciding whether or not a patient should have surgery. Chronic disease is a much stronger determinant of operative risk, but even patients with substantial medical comorbidity can successfully undergo major surgery. Management of the elderly patient poses many challenges to the physician and requires astute medical evaluation and optimal treatment of underlying conditions affecting risk. Postoperatively, special attention should be directed toward pain control, mobilization, mental status and prompt removal of indwelling catheters. Risk is not limited to the immediate perioperative period, requiring that management continue throughout the hospital stay and home settings.
