Paget-Shroetter syndrome. Case report

ABSTRACT

We present the case of a patient who attends presenting mild pain, edema, hyperemia, hyperthermia, tiredness and thoracic member heaviness, with history of heavy lighting exercise. Subclavian vein thrombosis was confirmed through a venous USG Doppler and phlebography, the treatment started with heparin and acenocumarin, and the patient underwent a extraction of the first rib 30 days after the beginning of the clinical picture, presenting proper evolution. The Paget-Schoetter syndrome (PSS) is an effort underarm or subclavia caused thrombosis, in young healthy patients without pathological history. It presents due to repetitive activity of the upper extremities, which is why causes microtrauma to the vascular intima and having the thrombus formation. The clinical picture includes pain, edema, paleness, marble-like skin and venous distension and phlebography. The treatment consists on thrombosis and/or early anti-clotting, as well as surgical recession of the first thoraxic cage rib.
