Prerenal failure on a cirrhotic patient undergoing umbilical hernioplasty

ABSTRACT
38 years old male patient with alcoholic cirrhosis admitted to the hospital due to spontaneous paracentesis through an umbilical hernia. The patient was intervened to perform a surgical wall repair. The hernioplasty was performed without tension with pre-peritoneal polypropylene mesh and a Penrose-type drain to avoid tension by the ascites fluid buildup and leakage. The patient was discharged after seven days of the surgery without complication, however the patient was re-admitted on the ninth day presenting nausea, diarrheic evacuations, and leakage of ascites fluid through the controlled fistula. Due to the high wear through the fistula in the post-surgical period, the patient developed prerenal failure and hypovolemic hyponanatremia instead of dilutional hyponatremia frequently associated to cirrhotic patients.REFERENCES
