ABSTRACT
It is here reported the clinical case of a masculine patient of 69 years old scheduled for phacoemulsification surgery. In this history we found hypertension and diabetes, he also suffered acute myocardial infarctation of one-year evolution and diabetic nephropathy. He was going to be operated upon, under ocular regional anesthesia (retrobulbar blockade) but as a retrobulbar haemorrahage was shown it was decided to cancel the procedure and chose a further date but under topical and intracameral anesthesia with 1% lidocaina preservatives free plus sedoanalgesia. The results were satisfactory, which demonstrates that topical and intracameral anesthesia is a good option that may be used when phacoemulsification surgery has to be performed in high risk patient, in whom, general anesthesia is to be discouraged.
