Systematic assessment of quality of life in patients with diabetes mellitus type 2 and diabetic nephropathy

ABSTRACT
Diabetes mellitus type 2 represents in Mexico a matter of concern in public health, as well as the complications that it generates as it is the chronic renal insufficiency (CRI) for diabetic nephropathy (DN); which originates an expense risen in their treatment and control by means of peritoneal dialysis and hemodialysis. Both illnesses have an impact so much physical as emotional along the chronicity process that the quality of people’s life that you/they suffer it deteriorates. For such a reason, the objective of the present investigation was the one of determining the impact of the deterioration of the quality of life in three samples of patient: Healthy, with diabetes type-2 and ND. It was carried out a comparative study of traverse type, in which is looked for to identify, to relate and typify their possible functional peculiarities. The fellows that participated in the study were 300 claimants of the Clinical Hospital "Dr. Francisco Padron Poyou”, of the IMSS of the capital of San Luis Potosí State, of the Services of External Consultation of Medicine Family and Peritoneal Dialysis, diagnosed by their owns doctors with diabetes type-2 (n = 100), diabetic nephropathy (n = 100) and healthy (n = 100), with similar socio-demographic characteristics. The deterioration of the Quality of Life was measured with the Scale of Quality of Life you Relate with the Health (ESCAVIRS), in its validated version and standardized to Spanish. The dependent variable to consider was the evaluation of the index of deterioration of the quality of life of the samples, dear through the scale ESCAVIRS. The independent variable understood the time of the evolution of the chronic-degenerative condition of the diabetes type-2 of the patients selected with diabetes and diabetic nephropathy, as well as some socio-demographic aspects. The results revealed a gradual and progressive process in the deterioration of their quality of life with significant tendencies as the illness advances in connection with the chronicity of the suffering. Deteriorate that goes from 5% in the healthy fellows, of 45% in those subject with diabetes type-2 and with 85% the patients with sequel of Diabetic Nephropathy . The areas of interaction stand out with the team of health, the sexual dysfunction, the emotional and physical aspects, among others. Having more risk in the deterioration, those patients with more than 5 years of evolution of suffering of the diabetes type-2, the educational level under, the age bigger than 50 years. We conclude to the degree of deterioration of the quality of life in this type of patient with diabetes type-2 and DN is progressive and associated to the degenerative evolution of the illness.
