Preoperative parameters predictive of mortality in the ruptured abdominal aortic aneurysm under the kidney in geriatric patients

ABSTRACT
Introduction: the ruptured abdominal aortic aneurysm under the kidney holds high mortality rates. There is not general consensus on identification of the preoperative parameters predictive of mortality from this cause.
Objective: to identify the preoperative parameters predictive of mortality from ruptured abdominal aortic aneurysm under the kidney.
Methods: a retrospective study was conducted on the patients who arrived to the "Saturnino Lora" provincial university hospital of Santiago of Cuba with a diagnosis of ruptured abdominal aortic aneurysm under the kidney from January 1st 1998 to December 31st 2008. The sample comprised 37 older patients. The study variables were age, sex, hemoglobin and creatinine concentrations, presence of electrocardiographic ischemia and state of consciousness.
Results: patients aged 60 to 75 years and males predominated. Twenty four out of the 37 patients died; all of them had some of the preoperative factors predictive of mortality that literature describes; for example, hemoglobin concentrations below 90 g/L in 23 (62.1 %) patients and myocardial ischemia also in 23 patients. Nine (37.5 %) patients lost consciousness.
Conclusions: low hemoglobin concentrations, cardiac ischemia and loss of consciousness are the most associates parameters to preoperative mortality in older patients. Their presence in one patient may predict his/her evolving into death.
