Medical and surgical management of peritonsillar abscess (QUINSY). Five years experience

ABSTRACT
Peritonsillar abscess is a severe infectious disease, been a common cause of consultation at our division, it’s actual management includes different modalities, among which incision and drainage, followed by antimicrobial therapy with penicillin and clindamycin are our choice. A clinical records review with peritonsillar diagnosis was done, from January 1st of 1996 through December 31st of 2000, including 117 cases in the present study. We measured the efficacy of medical and surgical treatment, the frequency of complications, and the number of tonsillectomies done in this group. An 88.4% efficacy for the surgical management was found. Of the total of patients, 89.7% received penicillin plus clindamycin as medical treatment, none of them requiring change of it. The only complication found was persistence of the abscess in 11.6% of the cases, all of which solved with a second drainage. Interval tonsillectomy was performed in 43.6% of the patients.
Conclusion: Incision and drainage posses a higher efficacy than other surgical options, the penicillin plus clindamycin combination can be considered a first choice antimicrobial combination. A relative low percentage of our patients accept to undergo tonsillectomy, placing then under higher recurrence risk.
