Congenital tuberculosis: report of three cases

ABSTRACT
Congenital Tuberculosis is characterized by hepatic granuloma and exclusion of postnatal exposure. Three cases are described: two with confirmed tuberculosis and another one by epidemiological suspect.
Case description. The first case is a 21 days masculine, he has multiple focus pneumonia without response to initial treatment, a Ziehl-Neelsen test was positive in a gastric aspirate.
A. The patient presented well response to specific treatment. His mother died by meningeal tuberculosis TB.
B. The second one was a 5 month female, she presented multiple focus pneumonia with a clinical response to unspecific treatment; she developed respiratory insufficiency 15 days later, she has no reactive PPD (Purified Protein Derivative) and tracheal aspirate with a positive BAAR (Acid-Alcohol Resistant Bacillus), she died 2 days after. The autopsy reveals hepatic, intestinal and lung granuloma.
C. The last case was a 5 month masculine, a previous case twin brother, with recurrent apical pneumonia. He started a mycobacterium-specific treatment after sister died with well outcome. conclusion. Probability diagnostic will be suspected with the clinical and epidemiological information in a low weight infant or with severe acute malnutrition with persistent pneumonia and a mother with suspected disease.
