Association of serum albumin with severity of diabetic retinopathy

ABSTRACT
Background: In patients with diabetes mellitus and endothelial dysfunction atherosclerotic changes are associated with the progression of chronic complications. In these patients has been observed low serum albumin and albuminuria determined by a chronic inflammatory process. Therefore, the objective was to study the association between serum albumin and the severity of retinopathy in Mexican patients with diabetes mellitus type 2.
Objective: Determine if serum albumin concentration is associated with diabetic retinopathy severity.
Material and methods: We included patients from hospitals of Secretaria de Salud del Distrito Federal with diagnosis of type 2 diabetes mellitus without renal involvement or hypertension. We determined the concentrations of serum albumin and sent the patients for ophthalmologic evaluation to detect diabetic retinopathy, and in his case, the severity of the same. Averages were calculated from serum albumin concentration and standard deviations of two groups: with and without diabetic retinopathy. Results comparison of the groups were performed by the Students t test and was considered statistically significant a
p-value ‹ 0.05. We included a total of 74 patients: 40 men and 34 women; average age of 49.7 ± 9.81 and 45.79 ± 11.01 years, respectively. We included 37 with and 37 patients without diabetic retinopathy.
Results: The duration of diabetes mellitus was higher among men (95% CI 3.85-8.84,
p ‹ 0.001 ) but the serum albumin concentration was lower in them than in women (95% CI 0.1451-0.6384 ,
p = 0.002 ). The higher prevalence of diabetic retinopathy was observed in men: 40 (57 %). The duration of diabetes differ in both groups (95% CI 4.56-9.48,
p ‹ 0.0001). The serum albumin concentration was 3.38 ± 0.558 g/dL in the group without retinopathy and 2.98 ± 0.4941 g/dL in the group with retinopathy (95% CI 0.582-0.6472,
p = 0.002). A serum albumin concentration less than 3 g/dL in patients with diabetes mellitus type increases their risk of retinopathy (OR 7.57 ; 95% CI 2.39-22.60). Sixteen patients had proliferative retinopathy compared with those without neovascularization. No differences were found in serum albumin concentrations, age or diabetes duration.
Conclusion. Less than 3 g/dL serum albumin in mexican patients with type 2 diabetes mellitus is associated with retinopathy.
