Tuberous breast: Etiopathology, classification and surgical treatment

ABSTRACT
There is a wide variety of malformations that affect the appearance and volume of the mammary gland, characterised by abnormalities on its base, particularly in the lower quadrants. The tuberous breast is the most typical, but not the only form of this abnormality. This paper deals with the possible causes of this basal mammary deformity. We report 29 procedures on 16 patients (13 bilateral and 3 unilateral), classifying them in four groups, depending on the severity of the malformation. In type I patients with hypoplasia of the lower-middle quadrant were classified; in type II the deficiency is in both lower quadrants, but with enough skin under the nipple-areola complex; type III differs from type II because of lack of skin under the nipple-areola complex, and in type IV we find severe mammary constriction with minimal mammary gland base. We propose a surgical procedure based on a periareolar approach, with a glandular flap based on a basal-inferior pedicle, with or without breast implant placement, according to the previous and/or existing volume.REFERENCES
