The last decade has seen great strides in the drug treatment of myocardial infarction and consequent heart failure, with associated improvements in quality of life, haemodynamics and survival. Despite this, the beneficits of pharmacotherapy are limited in more severe cases, and mortality and morbidity remain high. The ability to replace damaged myocardium by use of stem celI therapy offers a new and potentially exciting treatment modality, but its benefits are at present far from established. While clinical trials explore cells derived from skeletal muscle and bone marrow, basic researchers are investigating sources of new cardiomyocites, such as resident myocardial progenitors. Here, we briefly review the use of bone marrow derived and cardiac derived stem/progenitor cells in myocardial regeneration studies. We also discuss the challenge of translating basic cellular and molecular mechanism into effective, clinically relevant options.
