ABSTRACT
Introduction: Atherosclerotic popliteal disease is of great importance because appropriate blood flow below this area is paramount to the function and integrity of the leg and foot. The usual surgical procedures to correct this condition result in high morbidity as a result of knee joint flexure motions which make changes in the nature of the prosthesis element. Therefore, we have devised a technique that approaches the artery at the popliteal space.
Material and method: This technique was undertaken in 28 patients: 19 male and nine female, with an average age of 70 years, of age who were seen and studied at Ward 15 of the Angiology and Vascular Surgical Clinic in the Sanatorio Español in Mexico City. In all patients, the technique is based on the placement of a prosthetic that graft is implanted in the root of the superficial femoral artery in its proximal portion and at the distal end of the medial popliteal artery at popliteal space level after section and eversion and of same later use as the site of anastomosis.
Results: In twenty cases, many followed up for as long as 5 years, 50% remain with the bypass permeability.
Discussion: In our series, in patients at phases III and IV, success has been rewarding 50% patency in 5 years; therefore, we believe that it is worthwhile to communicate this technique that in our opinion is indicated in cases that comply with the indications as stated.
