Laparoscopic Cholecystectomy in Patients Age 60 Years or older. Report of a Tertiary level Hospital

ABSTRACT
Introduction: The experience is presented in the laparoscopic cholecystectomy in 61 patients of age 60 years in a 5-year period.
Material and method: Female patients were 41 (67.2%) and there were 20 males (32.7%), 37 patients (age range 60-70 years and 24, than 71 years of age, with elective surgery in 57 patients (93.4%) and emergency surgery, four patients (6.5%).
Concerning antecedents there were 15 patients with diabetes mellitus type 2 and 14 with arterial hypertension, and 20 with other illnesses.
Results: Four patients were diagnosed with choledocholithiasis by cholangiography and one case presented jaundice after 6 months of the operation.
Three cases had conversion to open surgery (4.9%) two had hemorrhaged in the hepatic channel, and in one case for billiary flight of the cystic one for bad staple placement.
Complications in seven patients (11.4%) were postoperative bradycardia, hepatic channel hemorrhage of the metabolic decompensation, and precordial pain.
Hospital stay was on average 48 h and return to daily activities was on average 7 days. Pathology reports in 45 patients were cholesterolosis and chronic cholecystitis, 10 with acute cholecystitis, three with purulent cholecystitis, and three with other diseases such as vesicular polyps and hydrocholecystitis.
Discussion: Patients 60 years of age present a greater risk of complications for metabolic decompensation; however, laparoscopic surgery diminishes this risk and this should considered as the surgical choice for vesicular pathology.
