ABSTRACT
Objective: To evaluate whether the use of antibiotics is justified in inguinal surgery that uses new or recycled polypropylene mesh.
Design: This was a clinical, prospective, comparative, longitudinal trial.
Site: General Hospital, 2nd level.
Patients and Method: Patients with primary or recurring inguinal hernia were treated surgically using new or recycled polypropylene mesh. They were divided into two groups: preoperative antibiotics were administrated to group A and not to group B. Patients were evaluated as to postoperative infection.
Results: In a period of years 184 inguinal repairs were done using polypropylene mesh in 112 patients with demographic and clinical similarities. The patients were divided into two groups according to their consecutive order of arrival at the hospital. Group A, 54 patients, received preoperative antibiotics and group B, 58 patients, did not receive antibiotics. The surgical techniques used were: Gilbert’s with cone mesh in 60 patients, Lichtenstein’s with mesh in 44 patients, and the preperitoneal approach with mesh in six patients. New or recycled mesh was used according to its availability at the time of the surgery. In 56 cases, new polypropylene mesh was used, in 26 cases the polypropylene was sterilized once, in 19 cases the mesh was sterilized twice and in 11 cases the mesh was sterilized three times.
There was no postoperative evidence of infection whether the patients received antibiotics or not or whether the polypropylene mesh was new or recycled several times. There was no evidence that the use of preoperative antibiotics in inguinal repair surgery using recycled polypropylene mesh was necessary.
