Toracoonphalopagus at the National Institute of Pediatrics. A case report

ABSTRACT
The siamese have fascinated the medical and general public since ancient times. Aristotle attributed the origin of the Siamese to the amount of semen and the shape of the belly. Hippocrates believed that the large number of «seeds» were responsible for multiple pregnancies and conjoined twins. Ambroise Paré described different real and imaginary cases of craniopagus, ischiopagus. The first documented case of conjoined twins was in England in the year 1100, they spent their life joined by their shoulders and hips. We report a case of conjoined toracoonphalopagus twins referred to a Hospital in Guerrero, a normal evolution pregnancy without prenatal care, upon entering the Institute ultrasound determined the presence of independent hearts with fused pericardium, Fallot tetralogy in one of the siamese, ectopy of the heart, independent livers joined by an isthmus and independence of other structures. Preparations are made to carry out a planned separation. 48 days after birth, however, 5 days after the separation there are hemodynamic repercussions causing cardiogenic shock. The case and interventions carried out in each one are presented, emphasizing that this case was the last one reported in our Institute in the past 10 years.
